Patient Policies & Information
We are committed to supporting your psychological and neuropsychological health. This summary provides key information about our services, your rights, and what to expect while receiving care. Please read carefully.
1. Scheduling, Cancellations, and No-Shows

Appointments should be scheduled in advance and confirmed.

Cancellations or reschedules should be made at least 24–48 hours prior.

Late cancellations or no-shows may result in a fee, which cannot be billed to insurance.
2. Telehealth Services

Telehealth sessions are conducted via secure, HIPAA-compliant video technology.

Telehealth can be convenient, but sessions may be interrupted due to technical issues.

You must be physically located in Florida during sessions.

Telehealth is not appropriate for emergencies. In a crisis, call 911, go to the nearest emergency room, or contact the 988 Lifeline.

You are responsible for attending sessions from a private location with a stable internet connection.
3. Fees, Insurance, and Good Faith Estimates (GFE)

Fees depend on the type of service (psychotherapy session, psychological testing, neuropsychological evaluation).

Insurance coverage varies; co-pays, deductibles, or non-covered services are your responsibility.

A Good Faith Estimate (GFE) is provided before services begin. It includes the expected cost range for your care.

If you are billed $400 or more above the GFE, federal law allows you to dispute the bill through our office or via the U.S. Department of Health and Human Services (www.cms.gov/nosurprises).
4. Informed Consent
Before beginning services, you will be asked to review and sign consent forms covering:

5. Confidentiality and Privacy

Your health information is protected under HIPAA and Florida law.

Information will only be released with your written consent, except in specific instances, which will be further discussed during intake.

You have the right to access, amend, or request copies of your records.
6. Emergency Situations

This practice does not provide 24/7 crisis services or care in urgent emergency situations.

In a psychiatric or medical emergency, call 911, go to the nearest emergency room, or call 988.
7. Communication and Contact

Clients may contact the provider between sessions for brief administrative or clinical questions, but extended clinical work (e.g., therapy guidance) is reserved for scheduled sessions.

Email and messaging through secure portals (like SimplePractice) may be used for appointment scheduling, billing, or document sharing, but are not guaranteed to be monitored continuously.
8. Record Keeping and Report Requests

All clinical notes, test results, and evaluation reports are part of the protected health record and maintained in accordance with HIPAA and Florida law.

Clients may request copies of records; raw test data may only be released to licensed professionals in accordance with ethical and legal guidelines.

Written authorization is required for release of records to third parties, including other providers, attorneys, or insurance companies.
